Before completing a registration form, please check that you live within our catchment area.
We may accept patients who live outside our area to register.
However if you live outside our boundary, some services and community care may not be available to you.
If you are unsure whether you can register, please contact the practice and we will be happy to help.
Registering with the surgery
To register with the surgery, please use the NHS register with a GP surgery: Sett Valley Medical Practice service or visit the surgery and one of our receptionists will help.
Please ensure that the digital registration form is fully completed, you will not be able to register without completing all mandatory information requirements. When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We do not require ID or proof of address to register you as a patient at the Practice, but should you wish to register for Online Access, we will need to see one form of photographic ID.
Named GPs
As one of our patients you have a named GP that is accountable for your care.
If you wish to know who your named GP is, please contact our reception team who will be happy to help. Having a named GP does not limit you to booking appointments with them; you are welcome to book with any GP of your choice.
Patient carer
If you are registered with our practice and you provide significant health or social care for a relative, please let us know so that we can ensure that you have access to the appropriate support for your role.
Registering as a temporary patient
If you are staying in the area and need to gain access to medical treatment you can register with the practice as a temporary patient.
You can be registered as a temporary patient for up to three months. Registering with the Practice as a temporary patient will not affect your registration at your regular GP Practice.
