Counselling
Appointments are arranged by the counsellor directly, following referral by the GP or self-referral.
You can make a self-referral by contacting one of the following organisations:

If you have been advised by the surgery to submit a Patient Health Questionnaire (PHQ-9) please complete our form.
Antenatal Clinics
Our antenatal clinics are on Tuesday afternoons. If you wish to attend please book an appointment and to confirm availability.
Maternity Clinic
These clinics are run by midwives. When your pregnancy is confirmed you should collect a pregnancy pack from the reception desk and complete the registration form, leaving it with the receptionist.
You will then be contacted by the midwife who will arrange your booking.
Vasectomy Clinic
Vasectomy Clinics are run at the surgery on Thursday afternoons.
Parentcraft
This service is run at New Mills Clinic in conjunction with the health visitors. To access this service please contact New Mills Clinic.
Audiology / Hearing Aid Assessment and Fitting
Patients over the age of 55 can self-refer for hearing aid assessment and fitting and no longer need to be referred by their GP.
Chesterfield NHS Service – patients have to ring 01246 514 403 to make appointment in a local clinic.

Specsavers @ Buxton
Patients over the age of 55 can use Specsavers as long as they have not been supplied with a hearing aid on the NHS within the last 3 years;(contact Specsavers on 01298 766 130 to make an appointment; you will need your NHS number to give to Specsavers).
Exclusions:
Patients under the age of 55 or with one sided hearing loss or complex hearing problems cannot self-refer for hearing aids need to see GP for referral.
